Quick
Application
We have
provided two options to submit the
information requested on the Quick Application
form.
Option
1
Once you have
filled out the Quick Application you may select the
Submit button at the bottom of the form and
the information will automatically be
forwarded for review.
Option
2
You
may fill out the Quick Application, print the form
on a local printer and then fax the form to 800-797-0578
(e-fax) for review.
A representative
from JG Consulting Group will be designated
to assist you with the completion of the
application process once we have reviewed the quick
application.
NOTE: Depending on the
version of Adobe Reader, Option 2 may be
your only method of submitting the
information.

2. Provide
as much information as possible.
3. Choose
one of the options above for form submittal.
